Jaw Fracture and TMJ disorders

During blunt force to the face, it is common for fractures to occur to the jaw. The jaw bone, or mandible is unique in forming an angle towards each ear which makes it vulnerable to fracture. Often the jaw bone is injured in two places. The initial fracture occurs at the site of the impact transferring the force of the impact along the thin jaw bone to the end of the jaw joint called the condyle. When this happens, TMJ disorders develop.

In children, fractures of the condyles are common due to falls. Often with children parents are most concerned about the obvious injury to the face as there is often a cut. The possibility of jaw fracture is not considered unless there are obvious signs such as misalignment, inability to chew, pain and swelling.

Parents who are concerned that there may be damage to the jaw, are wise to consult a qualified dental professional for an evaluation. Dr. Mizrahi can evaluate children for jaw fracture at her Los Angeles clinic and recommend a treatment plan if damage is found. Otherwise, children may develop future problems when the jaw fails to develop normally or they may refuse to eat foods that require force when chewing.

Some of the symptoms of jaw fracture are:

  • Normal bite is different or feels wrong
  • Misaligned teeth, loose teeth or missing teeth
  • Swelling or bruising in the gum
  • Pain in the jaw joint
  • Difficulty opening the mouth
  • Numbness in the lower lip or chin

Not all of these symptoms are indications of jaw fracture but should you or your child experience any of these, an evaluation should be done by a board certified oral and maxillofacial surgeon as soon as possible.

Methods of Treating Jaw Fracture

Treating jaw fracture has the goal of restoring a solid foundation in the jaw which allows normal function and the ability to chew solid food. When there are simple fractures, the jaw may only need to be wired closed to allow healing.

More serious fractures may need to be treated through incisions in the mouth or the face. In this case the bone fragments are realigned and secured with bone plates and screws. Arch bars are attached to the teeth to stabilize the teeth and stabilize the jaw while healing.

Healing usually is completed in 4 to 6 weeks unless the damage is extensive. In that case there may need to be several stages of rehabilitation to accomplish the goal of a stable jaw that functions normally.

After healing is verified as satisfactory, the wires and arch bars (if placed) are removed and rehabilitation can begin. Jaw exercises to help restore normal opening and closing are begun and patients can begin to eat soft solid foods.
